- a. Employment Certificate Original [must include position, date hired, compensation, office address, HR landline number (cell-phone number is not allowed), HR e-mail address] (Issued within 1-3 months of application)
- b. Photocopy of ITR (Income Tax Return) or Form 2316 Copy
- SELF-EMPLOYED:
- a. Business Registration from SEC or DTI (Copy), Business Permit or Mayor’s Permit (Copy) (For Corporation attached page with list of Incorporator or latest GIS)
- b. Photocopy of ITR (Income Tax Return) or Form 2316 / 1701 with corresponding payment receipt from BIR authorized bank and should include the pages with Net income and Total revenue
- Latest Original Personal Bank Certificate (Issued within 1-3 months of application)
- a. Must include account name, account number, account type, current balance, account opening date, 6 months average balance (ADB)
- Latest Original Personal Bank Statement (3 months) (Issued within 1-3 months of application)
- a. Original or certified true copy of bank statements/passbook for the last 3 months
- COVER LETTER: Execute a computerized, English, and signed letter of explanation for documents you cannot provide
